MANAGEMENT - Corona ( Covid-19 ) - Symptomatic Treatments
Corona ( COVID-19 ) management/treatment is done in 3 categories which is based on the severity of the disease
3 categories are:
- Mild
- Moderate
- Severe
- Mild cases are the cases in which the patients are either asymptomatic or may have mild symptoms like uncomplicated upper respiratory tract infections (spo2>94%room air, RR >24 breaths/minute, no evidence of hypoxiemia or breathlessness.
- Moderate cases are the cases in which patients may have pneumonia symptoms with no sigh of severe disease ( spo2 :94%-90% room air, RR: 24-30/minute)
- Severe cases are the cases suffering from severe pneumonia like symptoms with spo2 <90%room air, RR>30/minute)
- Critical cases are the cases having the symptoms of ARDS (acute respiratory distress syndrome).
- Sometimes critical cases end up with septic shock.
General measures advised to people are:
- Patients who are asymptomatic or having mild to moderate symptoms are advised for home isolation.
- So in home isolation one need to take necessary precautions like keeping oneself in isolation from other members of the family and need to have a pulse oximeter, thermometer and a spirometer with themselves.
- Taking proper diet is very much essential. Taking supplements like vitamin c, b complex and other multivitamins along with antioxidants is also essential .
Generally the doctors have been prescribing the home isolation kits that includes the things above mentioned and drugs like below:
- HCQ-400 BD on day 1 followed by hcq-200 BD day 2 onwards for 5 days in total.
- Doxycycline 100 mg BD for 7 days
- Ivermectin 12 mg OD for 3 days
Or
Tab. HCQ-400 BD on day 1 f/b 200mg day 2 onwards for 5 days in total
Or
Tab. Fabipiravir 1800mg BD on day 1 f/b 800 mg BD day2 onwards for 6 days (total 7 days)
Along the any one of the above medicines
- Apixaban 5mg OD for 7 days
- Zinc 50mg OD for 7 days
- Vitamin c TID for 7 days
- N Acetylcysteine 600mg (if patients having cough
- Dolo 650 to be used TID if high fever persists or else can be taken when there is fever
For moderate cases, along with the above treatment, steroids like below are taken.
- Methyl prednisolone 0.5 – 1mg/kg
Or
- Dexamethasone 0.1-0.2 mg/kg for 3-5 days
- Awake proning is very useful.
- Healthy diet is very much required for better recovery
- Hydration of the body is required as the fight with the virus and the effects of antibiotics/antivirals will ultimately lead to severe dehydration with in turn can lead to weakness.
- Plenty of water (2-3ltrs) to be taken
- For severe weakness ORS packets can be used if anything else isn’t helping.
- Cetirizine 5mg can be used for sore throat
- Warm water and steam inhalation also helps
But in case of severe and critical infections hospital admission is required. Then warning signals are:
- Spo2<90%room air,
- Shortness of breath
- Any chest pain.
DIET :
DIET includes the normal feeding without any restrictions. It can contain food rich in protein and fiber.
